   submitted with this application. (Photocopies are acceptable)
2. First year dues are currently $35.00. This includes $10.00 for the National Registry, $15.00 for
   the National Headquarters, and $10.00 remains with the Calgary Chapter.
3. Dues for each year after the first year are currently set at $25.00 and are due by the 1st of January
    each year. $15.00 is for National Headquarters, and $10.00 remains with the Calgary Chapter.
